River Valley Integrated Care

Kent County

River Valley is a 32-bed co-ed barrier-free program that specializes in treating adults with both medical and behavioral health needs.  The program provides long-term integrated care to persons with chronic mental illness who require a lower level of assistance than a traditional nursing home, including individuals with medical conditions such as:

  • Insulin-dependent diabetes
  • Multiple Sclerosis
  • Parkinson’s
  • Dysphasia
  • Traumatic brain injury
  • Huntington’s Chorea
Other program features include:
  • 24-hour direct care nursing supervision with a 1:3 staffing ratio on first and second shift, and 1:4 on third shift
  • Basic care and assistance with Activities of Daily Living (ADLs) provided by residential and nursing staff
  • In-house case management and behavior specialist
  • Weekly contacts with a psychiatrist, primary care physician, and dietician
  • Weekly in-house lab work, dental care, podiatry, and vision care
Programming consists of an active recreational day program which offers a variety of activities designed to give consumers access to the community, thereby increasing integration and encouraging socialization. The staff provide training and support in areas such as developing skills for symptom management, medication management and social skill development. Examples of other activities include: community meetings, bowling, bingo, weekly spiritual services, and music therapy. A wide variety of leisure time activities such as board games, cards, television, exercises, group activities, discussions and reading materials are also provided.

Case management services are provided internally to ensure that consumers receive annual person centered planning, as well as input into all other areas of treatment. These include monthly and quarterly reviews, future planning assessments, and behavior support planning.
