Walker Valley

Kent County

The Walker Valley program is designed to provide long-term, person-centered care, medication maintenance and day programming for older adults who have a mental illness. The program provides supports in order to maintain individuals in the community who might otherwise be in a more restrictive setting.

A Program Manager works with the Program Director in coordinating the overall administration of services to consumers, and ensuring that optimum licensing standards directly supervises the program and regulations are followed. Education and assistance to consumers is provided in developing skills for daily living, symptom management, medication management, cognitive skills, leisure and social relationships. The in-house day program emphasizes social skill development, daily living skills, pastoral services, and activities. Leisure activities and outings promoting community integration also occur on a regular basis.

Case management services are provided internally to ensure that consumers receive annual person centered treatment planning, as well as input into all other treatment areas.
