Supported Independent Living


Individuals who desire more independent community living options can receive supports in a variety of locations across West Michigan.  Our fully-trained staff provide support in various independent living settings and are trained to be sensitive to cultural diversity and strive to honor the individuality of each resident. Each resident is valued and respected as a whole person with physical, spiritual, and emotional needs.

Hope Network has access to connect you to affordable housing. Options include single-occupancy apartments, two bedroom shared apartments, and homes that are shared by 3-4 individuals. Support Services can be provided in our facilities or in your own home.

Our services are tailored to meet your individual needs and allow you to live as independently as possible. You are able to do as much for yourself as you can, while receiving the specific help and support you need. The services listed here are available based on your needs.
 

Basic services include assistance with:

  • Medications
  • Dressing
  • Toileting
  • Bathing and/or grooming
  • Laundry
  • Meal preparation or help eating
  • Shopping/errands
  • Money management
  • Crisis management

Additional services:

(Availability may depend on your location and available funding) 

  • Pastoral services
  • Nursing
  • Supports Coordination/Case Management
  • Training
  • Psychiatric services
  • Transportation to appointments

Community living support services are an integral part of your overall service plan. We will work with your family, friends and other service providers, like your Supports Coordinator or Case Manager, and participate fully with your support team to provide the most comprehensive service possible.

Funding options may include private payment, Department of Human Services, Community Mental Health agencies, Special Needs trusts, or private insurance. Funding by DHS or CMH agencies may require authorization/approval.

Hope Network Workforce Development has partnered with Volunteer Income Tax Assistance Program (VITA) through Davenport University, to be a host site for free tax preparation services. This service is available to all Hope Network staff, consumers and the public.

Income tax assistance is available on Fridays, beginning February 3rd, from 9:00 a.m. to 7:00 p.m. at the Education Center in Grand Rapids.

Hope Network hosted a special book signing of the new book, 'The Book of Cake' a collection of Cake stories from the past year which feature Hope Network consumers, employees and volunteers. 

Award winning writer Tom Rademacher, who writes the stories for Hope Network,  addressed the crowd of employees, Board Members and many of the people featured in the stories.

Heart of West Michigan United Way invites you to take on the Small Business Challenge, created through a generous matching grant of $100,000 from the Steelcase Foundation. Any new United Way gift of $1,000 or more from a workplace of 100 or less employees will be matched up to a total of $100,000 by the end of March 2012.
 
Small businesses are the heart of most communities, yet during the 2010 United Way Campaign small businesses of 100 employees or less made up only 11.5% of total campaign dollars. This campaign year, United Way is hoping to change that with the Small Business Challenge.
