Program Description

Population Served

Persons with severe disabilities as defined by the Ability One Program requirements. 

Type of Services Provided

Currently janitorial and grounds services to Federal Government customers in Metro Grand Rapids.  Program is looking at other service lines of business including; tele-services, secure mail-document services, laundry services, fleet management services.

About the Service

Provider Contracts have been managed by Hope Network staff for more than 25 years focusing on training and high customer service. 

Purpose and Goal of Services

Outcomes (job placement, maintain quality of life, community integration)

Successful Exit Strategies

Performance improvement resulting in community placement in similar career or improvement in disabling condition whereby it is no longer considered a severe disability.

Environment

Janitorial work is completed in a variety of settings including offices, restrooms, air traffic control tower, museum, court rooms, classrooms, weight rooms, medical clinic exam rooms etc. Grounds maintenance is completed for two downtown locations in a variety of weather conditions including snow, rain and extreme heat on occasion. 
 

Entrance Criteria

  • Initial disability determination including a signed statement by a licensed health care provider qualified to provide diagnosis of disabling condition.
  • Referral type – Severe physical or mental impairment that limits one or more functional capabilities (mobility, communication, self-care, self-direction, work tolerance, or work skills) resulting in the referral being unable to engage in normal competitive employment over an extended period of time.
  • Need for service – Job training and/or hardening, placement goals (IWPP)
  • Funding – Not required unless additional job coaching is needed to start.
  • Can function in physical environment, ambulatory, multi step directions.
  • Ability to access services - Own transportation or ability to ride city bus.

Exit Criteria

  • Meets stated goals and can move to greater independence including competitive employment.
  • No longer meets entrance criteria (health status change, no longer considered severely disabled according to Ability One criteria)
  • Lack of compliance with contract customer requirements for quality performance or behavior.
  • Self Termination

Applied Behavior Analysis for Autism Spectrum Disorders

Grand Rounds is a new series of professional lunchtime lectures covering a variety of topics relating to autism spectrum disorders and other neurodevelopmental conditions, including ABA, Speech and Language Pathology, and supporting children and adults with autism. Lectures are free of charge and open to the public. Guests are encouraged to bring their lunch. Following each lecture, there will be an opportunity for questions and answers.


All lectures are from 12:00-1:00 p.m. at the Hope Network Education Center, 775 36th Street SE, Grand Rapids, MI.
